Health Care Accreditation Process
Accreditation Canada’s Qmentum and Qmentum International accreditation programs are more than an on-site survey. They’re about quality improvement, every day, with everyone that walks through your door.
During the cycle, your organization is evaluated against best-in-class standards established by our affiliate, Health Standards Organization (HSO).
You will be assigned a dedicated Accreditation Specialist, who will guide you through each accreditation cycle. Your Accreditation Specialist is available every step along the way to answer any questions and to provide you with the resources you need to successfully achieve and maintain accreditation.
Cycle length ranges from 3-5 years depending on your jurisdiction and the type of care you provide

Early in your accreditation cycle, you will conduct a self-assessment questionnaire across your organization. The self-assessment will provide you with information on how your organization is currently meeting the required standards, identifying strength areas and gaps.
Measure, Improve, Prepare
Use tools such as questionnaires to assess your organization’s work life culture, patient safety culture and governance functioning, helping to identify more specific areas for improvement before your survey.
The simulated on-site survey will prepare you for the accreditation survey by helping you become familiar with the methodologies used by Accreditation Canada’s surveyors.
A team of peer-surveyors will visit your site to evaluate your organization’s clinical and administrative processes. The surveyors will talk with and observe patients, families, staff and leadership and offer real-time feedback and recommendations based on best practices.
Report, Decision and Action Plan
Following your on-site survey, you will receive a detailed report and an accreditation decision. If further action is required, we will support you and conduct a follow-up review to verify evidence of action taken and to remove any outstanding conditions from your accreditation decision. Your Accreditation Specialist will also provide you with the resources, tools and support you need to build and maintain a culture of continuous improvement in your organization.
