The Qmentum Accreditation Process

The Qmentum accreditation program is more than an on-site survey. It’s about quality improvement, every day, with everyone that walks through your door.

During the cycle, your organization is evaluated against best-in-class standards established by our affiliate, Health Standards Organization (HSO)

You will be assigned a dedicated Accreditation Specialist, who will guide you through each accreditation cycle. Your Accreditation Specialist is available every step along the way to answer any questions and to provide you with the resources you need to successfully achieve and maintain accreditation.

Accreditation Cycle

Cycle length ranges from 3-5 years depending on your jurisdiction and the type of care you provide. For more information, please contact us.


Early in your accreditation cycle you will conduct a self-assessment questionnaire across your organization. The self-assessment will provide you with information on how your organization is currently meeting the required standards, identifying strength areas and gaps.


Measure, Improve, Prepare

Use tools such as questionnaires to assess your organization’s work life culture, patient safety culture and governance functioning, helping to identify more specific areas for improvement before your survey.


On-Site Survey

A team of peer-surveyors will come to your site and evaluate your organization’s clinical and administrative processes. The surveyors will talk with and observe patients, families, staff and leadership.

In addition to receiving an objective, third party evaluation of your quality and safety, our peer-based team of accreditation surveyors offer valuable feedback and recommendations based on best practices they’ve seen employed in other organizations across Canada and the world. This is also an opportunity for you to share your own innovative practices with surveyors, who are then able to share what you’ve done on future surveys with other organizations. Learn more about our surveyor team.



Following your on-site survey, you will receive a preliminary report that includes initial findings. Your Accreditation Specialist will confirm with you the details of the report and ensure you and your staff understand it fully.


Decision & Action Plan

At this point you will receive your accreditation decision. If there are any action items, your Accreditation Specialist will work with you to ensure you understand them. They will provide you with the resources, tools and support you need to build and maintain a culture of continuous improvement in your organization.
